Pine Haven is a 70-bed skilled nursing facility on a 5.8-acre campus. Evergreen Place is connected to Pine Haven and offers 24 assisted-living units on 1.02 acres.
The seller of Pine Haven was the USDA. The City of Pine Haven sold Evergreen Place. The buyer is a regional operators with other communities in Minnesota.
